Care Connect is a major community care provider that supports people to live healthy, independent lives at home and in the community.

With offices in NSW, QLD and VIC, Care Connect supports children, young people, special interest groups, adults, carers and older adults. The organisation offers people access to information services and support, assessment and planning, case management and care coordination, advocacy, service delivery, and carer support.

Care Connect's approach to community care is collaborative, inclusive and accessible. Staff members are committed to providing a respectful service that values the rights of each person and their support networks. Of prime importance is building each person's capacity and confidence in leading an independent life at home and in the community by ensuring support requirements are identified, met and monitored.

Care Connect's specialist in-home support-teams can help people navigate the health system, assess a person's needs to determine which services and supports they require, and tailor an individual support plan to suit individual requirements.

Care Connect specialises in support and services relating to Acquired Brain Injury (ABI), aged and disability, Autism Spectrum Disorder, children's and young people's packages, dementia, housing support, Indigenous services, and preventative and transitional care.

Care Connect is a not-for-profit, non-denominational, registered charitable organisation. Care Connect receives funding from the Commonwealth Government, Victorian Government and NSW Government to provide community-based services.
